You really don't need a recipe, more a set of guidelines. There is no need for supplements and these can in fact cause problems with oversupply of nutrients. Feed approximately 70-80% meat, 10-15% edible bone and 10-15% offal (including liver). These ratios will supply your dog and cat with everything they need to be fit and healthy. As Bindi suggested start with 2-3% of expected adult weight per day and go from there. Some dogs need less and some more food to maintain a healthy weight. Don't forget that most of the dogs we see are actually overweight so use your hands as well as your eyes. If you can feel your dogs' ribs under the skin this is good!! If they feel like a washboard then feed more - if you have to press to feel them then feed less!!
These sites are all great sources of information. I would also recommend joining one of the yahoo raw feeding groups - there are several but I feel that raw feeding and RawMeatyBones are the most helpful.
If you can, get hold of Dr Tom Lonsdale's book 'Work Wonders' as this is a great resource too.

You need to feed 2-3% of his expected adult weight daily.
Start out with one protein source such as chicken, chicken leg quarters would be a good starter. Use that for a week, then add another protein source such as pork, beef, or turkey.
Variety is the key with a raw diet. Its easy once you get the hang of it but its not as simple as throwing a couple chicken legs in a bowl everyday.
